Enrollment Process
If you are interested in learning more about MHOH Child Development Center, visiting our facilities or enrolling your child, please contact us at (937) 951-2547.
The enrollment process requires you to complete the following:

Interview with the Director or Administrator

Tour of the facility

Complete the enrollment forms for ALL PROGRAMS or PRESCHOOL.

Preschool Enrollment ~ Additional Requirements:

Child/ren must be 3 or 4 years old by October 1st

Complete the ECE Grant Preschool Application
-
Most recent w2s, 1040, or last 4 paystubs
-
Verification of residence: utility bill, lease agreement
-
A copy of the enrollee’s birth certificate
-
Last 4 digits of the enrollee’s social security number
We offer Enrollment for the following:

Title XX
